Educating our patients can help reduce health care costs, lost work days and improve quality of life.

Our disease management program supports patients with chronic conditions. Specific programs address asthma/chronic obstructive pulmonary disease (COPD), diabetes, HIV management and osteoporosis prevention and management.

Our case managers help maintain or improve health by educating and coordinating services for high-risk patients through our case management programs.

Over the years Brown & Toland has earned numerous awards for quality and earned the prestigious "Blue Ribbon" award for medical excellence form the Pacific Business Group (PBG) three years in a row. We actively support and participate in the industry's payor initiative, "Pay for Performance," and we are deeply committed to our programs for best practice medical management of congestive heart failure, asthma, diabetes, HIV, and osteoporosis.

Newt Gingrich acknowledged our commitment to innovation at the State Senate Hearting of the Subcommittee on Technology, Innovation and Competitiveness in Washington, D.C. Brown & Toland was one of the first medical groups to introduce an electronic health record for physicians. In May of 2005, Brown & Toland introduced ihealthrecord, a personal health record for all residents of Northern California. Most recently Brown & Toland finished in the top 10 percent of all participating medical groups and IPAs in California.
